Return Goods Policy
1. Authorization
All returns must be authorized by SentraMed, LLC prior to receipt. Products must be returned within 60 days of the invoice date. Return Goods Authorizations (RGA) are valid for 15 days. Unauthorized returns will be refused, and no credit will be issued.
2. Return Procedure
After obtaining an RGA number via support@sentramed.com, the return must include:
- Customer name and account information.
- RGA number clearly marked on the shipping label.
- Original Purchase Order (PO) or SentraMed Invoice number.
- Lot number and expiration dates (if applicable).
3. Non-Returnable Items
The following conditions will not be considered for return:
- Products purchased more than 60 days prior to the return request.
- Special or custom-made products (non-stock items).
- Products with altered, opened, or damaged original packaging.
- Items with less than 6 months of shelf life remaining.
- Refrigerated or hazardous materials.
4. Damages, Shortages, and Shipping Errors
Inspection: Customer must inspect and count all items upon delivery.
Notification: Any damages, shortages, or shipping errors must be reported to support@sentramed.com within three (3) business days of receipt.
Documentation: Proof of damage (photos and a signed delivery receipt noting the damage) is required for any credit or replacement claim.
5. Restocking Fees (Standard Items)
For non-defective returns, the following fees apply to cover administrative and inspection costs:
| 0 – 30 Days | 15% restocking fee + All Freight Charges. |
| 31 – 60 Days | 25% restocking fee + All Freight Charges. |
| Over 60 Days | Non-returnable. |
